This 45 yrs old patient developed paralysis in all 4 limbs after an accident. Since the touch sensation was preserved we had some hope for his recovery. MRI was showing an altered signal in spinal cord and no cord compression which led to the diagnosis of a whip lash injury .On a closer look we found that his spine was not properly aligned and suspected an instability which was confirmed with dynamic Xrays. His spinal cord was swollen significantly and early surgery would have been more damaging .We treated him conservatively with advanced physiotherapy for next 3 months till he recovered significantly. His surgery was then planned and executed by anterior Decompression of spine and fusion by bone graft followed by fixation. Patience is the key in neurosurgery and performing surgery at a right time gives wonderful results.
